    There was a change done to address a potential vibration problem. To his
    credit, Sam Johnson sent the mod out to all previous owners. Mine came with
    it.

    Don't know if this is what happened to your friend. A 16" or 150 lb scope is
    the stated limit for the type V. Maybe that is a factor as well.
